We value health equity, inclusion, community engagement, accountability & transparency, excellence and collaboration & partnerships.</p><p><strong> </strong></p><p><strong>Position Summary: </strong></p><p>The Social Worker will work as part of a community-based interdisciplinary team to provide mental health and psychosocial care, therapeutic counselling, case management and follow-up support to clients within the context of a client-centered care model and under the established standards of professional practice. We are seeking a Social Worker who is committed to strength-based care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to community resources and comprehensive health care.</p><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p><p><strong>Main Tasks and Responsibilities</strong></p><p></p><p><strong>Client Care</strong></p><ul><li>Provide assessment and counselling services with individuals and families including children, youth, and adults.</li><li>Jointly work with the client and interdisciplinary team to develop a service plan outlining therapeutic goals appropriate to the client?s needs and issues.</li><li>Provide information, referrals, basic case management and client advocacy when needed.</li><li>Facilitate client linkages with internal/external resources and effectively coordinate client supports.</li><li>Attend and/or coordinate case conferences as needed.</li><li>Maintain clinical treatment records for each client and prepare verbal and written reports and correspondence related to the work.</li></ul><p></p><p><strong>Group Work and Community Development</strong></p><ul><li>Conduct health education/promotion workshops on mental health and wellness and other issues based on the needs of the community.</li><li>Participate in community-based groups or committee relating to Social Determinants of Health.</li><li>Participate in outreach activities as needed.</li><li>Conduct and complete group program evaluations.</li></ul><p></p><p><strong- Location North York, ON
