Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
*The opportunity*
We currently have a full-time permanent Social Worker opportunity at March of Dimes Canada (MODC). The Social Worker works within the Community Support Services program at MODC assisting clients navigating complex service issues and conflict to maximize their opportunity for success in our programs. Through therapeutic interventions, the unique needs of our clients will be balanced with the scope of our services and the staff team, who play a key role in supporting quality outcomes. This role also provides oversight of our applicant/client assessment and intake function.
*Responsibilities*
* Work with clients individually or in group situations who are presenting challenges to benefit from or understand the intent of service delivery models to assess their situation and identify the type of service or intervention that will support a quality outcome.
* Build effective rapport with a variety of clients to assist with facilitation of a positive outcome related to service delivery.
* Work with various staff teams on solid and consistent action plans for complex clients.
* Lead/Facilitate conflict mediation for client-related situations.
* Serve as a resource to assist the team to address complaints or internal investigations as appropriate.
* Provide case review and case conferences or training to staff to better support emotional coping, behavioral response difficulties being experienced by clients.
* Conduct psychosocial assessments for complex and difficult-to-support clients.
* Offer counseling to help clients navigate grief, loss, and trauma. Assist clients in developing skills to deal with and resolve their social and personal concerns.
* Research suitable community resources to further address/support complex client needs.
* Ensure that interventions and outcomes are in accordance with organizational policy and procedures. Evaluate the effectiveness of interventions and determine the need for additional team or client guidance.
* Work collaboratively within an interdisciplinary team of both regulated and unregulated health professionals.
* Act as a resource to staffing groups, including the Community Support Resource Workers, Supervisors, and front-line teams.
*Qualifications and experience*
* Masters of Social Work or equivalent
* Demonstrated understanding of disability
* Community-based experience working with seniors and individuals with disabilities, preferably 1-3 years experience
* Excellent interview and assessment skills
* Strong conflict mediation skills
* Strong boundary and relationship skills
* Excellent communication skills
* Demonstrated skills in assessments, treatment, individual and group counselling
* Experience/training in a variety of evidence-based therapies such as: solution-focused therapy, EMDR, CPT, CBT, DBT, and group therapies
* Knowledge and skills in a range of theoretical perspectives, psychosocial rehabilitation principles, self-management, and holistic wellness approaches
* This role requires a demonstrated appreciation of client-centred, strength-based care including the importance of accessibility, accountability, best practices, and evaluation
* Registered and in good standing with the Ontario College of Social Workers and Social Service Workers
