*TORONTO GRACE HEALTH CENTRE: CAREER OPPORTUNITY* *ADVANCED PRACTICE NURSE ? PERMANENT FULL TIME* The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites ? the Main site (Church& Bloor St), Harbour Lights and a Specialized Care Centre (Victoria Park & Mc Nicoll). This position will be required to travel to all sites of Toronto Grace. *POSITION SUMMARY* Reporting to the Chief Nurse Executive, the Advanced Practice Nurse (APN) will provide expert consultation to support the professional development of staff and the provision of high quality, evidence informed and safe care to complex patients across all programs at TGHC, from admission to discharge. *MAIN RESPONSIBILITIES* * The APN utilizes educational preparation, in-depth nursing knowledge and expertise in meeting the education and health needs of individuals, families, groups, communities and populations. * More specifically: * Understanding , interpreting and applying nursing theory and research methodology * Developing and advancing nursing knowledge and the profession as a whole, providing leadership and promoting excellence inpatient focused clinical care in collaboration with the patient, family and inter-professional team ( CAN, 2008) * The APN will provide leadership, mentorship, education and support to the nursing, medical and health discipline staff. * Conduct research and implement quality improvement initiatives * Additional duties as assigned *EDUCATION AND QUALIFICATIONS* * Master?s degree in Nursing or other health related science required * Undergraduate degree in Nursing from an accredited University * Current registration with the College of Nurses of Ontario * Canadian Nurses Association Specialty Certification preferred * 3-5 years recent clinical experience in geriatrics, internal medicine, palliative care, critical care and other specialty areas of care (Clinical Nurse Specialist or APN experience preferred) * Knowledge of statistics and ability to analyze and synthesize fiscal and patient care data * Experience in developing clinical tools such as policies/ procedures, clinical protocols, order sets * Experience in education program planning and evaluation * Knowledge of curriculum development, and teaching learning strategies * Excellent interpersonal, communication, collaboration, leadership, organizational, problem solving and decision making skills * Knowledge of relevant legislation and regulations * Microsoft Office skills * Exceptional time management skills with ability to multitask and prioritize * Self-motivated, team player, and ability to thrive in a fast paced environment. * Excellent attendance record, ability to work independently. * High level of integrity and emotional intelligence * Leads by example and exemplifies our Core Organizational Values. *Qualified candidates may apply to:* To apply for this exciting career opportunity, please email your resume and cover letter to jobs@torontograce.org referencing job: *05312024-* *Advance Practice Nurse Full Time*.
