Position #: 205948 Expected Start Date: December 02, 2024 Union: SUN Facility: Candle Lake Primary Health Care City/Town: Candle Lake Department: Primary Care Sites Type: Full-time regular FTE: 1 Shift Information: Days Hours of Work: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Field Hours: Yes Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: Yes Job Description: IN COLLABORATION WITH PHYSICIANS AND THE PRIMARY CARE TEAM, THE NURSE PRACTITIONER WILL PROVIDE PRIMARY HEALTH CARE SERVICES TO SEVERAL COMMUNITIES. THIS INCLUDES: HEALTH PROMOTION AND PROTECTION, DISEASE AND INJURY PREVENTION, PROVISION OF SUPPORTIVE, INSTRUCTIVE, TREATMENT AND EMERGENCY SERVICES; COLLABORATION WITH AND REFERRAL TO OTHER HEALTH CARE PROFESSIONALS; CONDUCTING COMMUNITY NEEDS ASSESSMENTS AND PARTICIPATING IN EVALUATIVE PROJECTS. THE INCUMBENT REPORTS TO THE DIRECTOR OF PRIMARY CARE FOR THE PRINCE ALBERT PARKLAND HEALTH REGION.
