Description PeaceHealth is seeking a Admissions Coordinator: Care Management for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $22.70 - $34.04. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Job Summary Responsible for identifying potential swing bed candidates, pre-screening referrals and preparing patients for admission to the swing bed program. Responsible for assistance with discharge arrangements for care facility and home discharge.Provides support to the Care Management Team to ensure that the day-to-day operational functions of the department are completed in a timely manner.
