As a Social Worker with Vancouver Coastal Health you will: Provide case management services and clinical and consultative social work services to individuals with complex mental health, addiction and chronic medical concerns or equivalent. Conduct biopsychosocial mental health and addictions assessments, provides counselling services, maintains client records, and attends interdisciplinary clinical/case conferences.
