*The main purpose of this position*
The Nurse Practitioner (NP) will provide in-person and virtual access to primary care services to vulnerable populations in Halton, including those with complex health care needs, mental health and addictions concerns and unhoused. The position will work as a team member of the Interprofessional Primary Care Outreach Team. The NP is responsible for ensuring that nursing care provided to clients meets all legislative requirements and practice standards of the College of Nurses of Ontario (CNO).
*The main purpose of the Enhanced Primary Care Pathways and Integrated Care (EPPIC) Team*
The EPPiC Team is responsible for providing one-to-one community-based primary care and clinical support across the Halton region supporting people who are unattached to primary care, unhoused and who experience complex health care concerns including mental health and substance use concerns, including Hep C, HIV/AIDS etc. The team will work primarily in the community meeting people where they are at, including shelters, supportive housing, street level outreach and key community partner locations. This team will include Nurse Practitioners, Registered Nurses, Social Workers, Peer Support Workers and an Occupational Therapist.
*Responsibilities*
_Client Focused Work_
* Delivers primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments, physical examination, therapeutic management, consultation, collaboration, and referrals for those who use substances.
* Assesses, investigates, diagnoses, and treats patients, utilizing critical thinking and evidence-based medicine and nursing as a guide to practice.
* Provides and evaluates holistic patient care based on advance nursing and medical management knowledge and skills.
* Discusses diagnoses with the client, including discussing relevant clinical information, treatment and the expected outcomes and prognoses.
* Work with clients and their circle of care to develop a plan of care (Unified Care Plan) utilizing assessment findings, diagnosis, and evidence-informed practice.
* Establishes a plan to monitor the client?s response to medication therapy, including withdrawal management and continues to adjust a medication/treatment and support based on assessment of the client?s response.
* Consults with physicians for clients who have unstable/complex health conditions.
* Performs physical exams, orders tests, diagnoses and treats substance use disorders e.g., prescribing withdrawal medications specific to their substance(s) of choice, safe supply and or anti-craving medications.
* Writes prescriptions and orders laboratory tests within the NP scope of practice
* Provides supportive counselling/short-term crisis management
* Conducts essential home visits to clients living within the Halton Region
