Precision Care Centre is hiring!
Precision Care Centre is focused on primary health care with utmost attention and professionalism. We are currently seeking a Full-time Nurse Practitioner to join our team at both of our clinic locations in Brampton, ON and Hamilton, ON.
The Nurse Practitioner (NP) reports directly to the Clinic Medical Director and is also accountable for meeting the primary term care policies, procedures, and protocols for medical services. The NP is an autonomous primary care provider to patients and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario. The NP collaborates with the staff and, and the health care team in the development, implementation and evaluation of the Walk-in plan of care. The NP must have valid CNO registration and First aid CPR/BLS certificate.
*Responsibilities include but are not limited to: *
-Provides comprehensive primary care to patients as part of the interdisciplinary health care team.
-Conducts person-centered comprehensive health histories.
-Engages patient s in dialogue to determine what is important to them for health and quality of life.
-Provides person-focused health education.
-Manages the care of patients by providing pharmacological, complementary and/or counselling interventions, and performs procedures within the NP scope of practice.
-Orders and/or performs appropriate screening and diagnostic investigations, interpreting results, and assuming responsibility for follow-up.
-Diagnoses acute and chronic health conditions.
-Collaborates and consults with physicians, the Medical Director of Nursing, nursing staff, interdisciplinary team members and external resources regarding a patient's plan of care.
-Makes referrals to specialized consultants, services, and other health providers.
-Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, patient responses, and clinical rationale in a timely and accurate manner.

Experience: Nurse Practitioner - 1 year (preferred)

Pay: $50.00-$70.00 per hour
Schedule: Monday to Friday
Education: Bachelor's Degree (preferred)
Licence/Certification: CNO Membership (required)
First Aid Certification (required)
Valid ACLS (required)
BLS Certification (required)
CPR Certification (required)
