Requisition Details: Employment Status: Permanent, Part-time (0.6 FTE) Program: Occupational Therapy, Community Aging in Place, Advancing Better Living for Elders (or CAPABLE) program Work Schedule: DaysNumber of Hours Bi-weekly: 45Service Area: Kings CountyThis opportunity requires working with clients in their home. Travel is required within the service area. When not delivering services, the Occupational Therapist may work from home and/or the VON office. VON is currently seeking Occupational Therapists to join the CAPABLE team!Occupational Therapists will be supporting the Community Aging in Place, Advancing Better Living for Elders (or CAPABLE) program which supports older adults to improve safety, function and independence so they can remain living in their own homes and communities with customized modifications. You will support the NS CAPABLE program, the first of its kind in Canada. Online training and support are provided to become a certified CAPABLE clinician. Key Responsibilities: Demonstrates knowledge, skills and attitudes as outlined in the Occupational Therapy standards and guidelines for evidence-based practice in order to assess, plan, and evaluate progress (for potential discharge) and document a patient plan of care within a patient-centred approach to therapy goalsAssesses the client's ability to function safely in the home/community environment Assesses clients from a holistic approach, i.e. mental health status, social contacts, and spiritual and physical needs that may potentially impact the patient's ability to maintain independence in a home environmentDevelop a client-centred plan with the client and healthcare team Provides education and recommendations to clients pertaining to the therapy goalsMonitors client outcomes and progression toward therapy goalsModifies client's care plans as required based on patient outcomesDocuments client outcomes and current care plans in the electronic patient chart according to the funder and College of Occupational Therapists of Nova Scotia guidelinesCompletes and updates the written care plan (electronic copy) with information collected from the patient, his/her family and any other healthcare or educational professionals involvedDemonstrates professional code of conduct/communication with all members of the health team as per VON guidelines and policyEducation, Designations and Experience: Masters in Occupational Therapy (or degree prepared occupational therapy education prior to 2008)Valid registration with COTNS and associated liability insuranceMembership with the Nova Scotia Society of Occupational Therapists is preferredExperience in a health care environment (i.e. long-term care, acute medicine, rehab unit, rehab clinic, community visiting program)Demonstrates knowledge of community resources preferredOther: Must have a personal vehicle and possess both a current driver's license and proof of vehicle insuranceA current and original copy of a satisfactory Criminal Records Check is requiredA Vulnerable Sector Search and/or Child Abuse Registry Check may be requiredMust be able to wear Personal Protective EquipmentWhat we can offer you: Opportunity to become a certified CAPABLE clinician Competitive salaries and vacation timeTiered Benefits (health, dental, LTD) & an Employee Assistance ProgramDefined Benefit Pension Plan with company contributionTravel compensationComprehensive orientation programSMART phone provided to keep you connected during work timeThe opportunity to make a difference in your communityCome for the job, stay for the learning, and the chance to make a difference.
