This is a senior role that reports directly to the Administrator. The level of knowledge, skills and abilities for this position are typically acquired through the completion of industry appropriate academic credentials, including:
* A Baccalaureate Degree in Nursing.
* Current Nurse Practitioner Registration with the College of Nurses of Ontario in the Extended Class, with a preferred Master?s Degree and the ability to prescribe narcotics and controlled substances.
* A minimum of 2 years clinical experience in the care of geriatric clients and adult chronic disease management.

*Key responsibilities associated with the role include*
* Collaboration with the resident and family/caregiver, and the health care team in the development, implementation and evaluation of the resident?s plan of care.
* Provision of leadership and mentorship to LTCH staff that enhances their knowledge, assessment skills, and ability to care for residents in place.
* Leadership and collaboration in research, education, and evidence-based practice initiatives to optimize the resident, LTCH and health system outcomes.
* Participation in providing after hours and on-call coverage in accordance with the on-call policy of the LTCH.
* Advocating for and providing palliative and end-of-life care.
* Assessment and ongoing monitoring of residents with complex care needs and/or altered health status.
* Promotion of innovative thinking and practices in the care of residents through the sharing of new trends in geriatric care.
* Functioning within and supports the mission, philosophy and values of The Elliott Community, contributing to future goal planning.
* In collaboration with the interdisciplinary team members, review and analyze data for trends, coordinate quality improvement initiatives and guide corrective actions in order to maximize service and minimize resident risk.
* Engaging in evidence-informed practice by critically appraising and applying relevant research, and theory in providing health care services.

* This is a permanent opportunity, with flexible hours (30-35 hours weekly), primarily days but some flexibility may be required.
