*Job Purpose:*
The *Homebase Program* is an intensive case management initiative, run in partnership with other local non-profit groups, targeting individuals and families experiencing chronic homelessness. Homebase Outreach Workers play a vital role in supporting housing-first clients by helping them overcome barriers to permanent housing.
As a Homebase Outreach Worker, you will be responsible for case management, record keeping, and providing ongoing support for clients within our outreach programs. You?ll work closely with the Homebase Task Force, landlords, and other agencies to ensure a coordinated approach to client care.
*Key Responsibilities:*
* Work collaboratively with the Homebase Task Force, staff, and volunteers to coordinate services for community members.
* Meet all contractual obligations and adhere to practice standards.
* Create and maintain detailed client documentation and records.
* Help clients access financial, medical, educational, and mental health support services.
* Participate in bi-weekly case review meetings and monthly task force meetings.
* Build partnerships with other agencies, supporting cross-agency case planning.
* Supervise and mentor practicum students in the outreach program.
*Qualifications:*
*Education:*
* Degree in Human Services or a related field preferred.
* Candidates with post-secondary education and relevant experience will also be considered.
*Skills, Abilities, and Attitudes:*
* Strong commitment to The Olive Tree Community Centre?s vision and mission.
* Excellent listening and communication skills; ability to motivate teams and respond to individual and group needs.
* Strong attention to detail, with the ability to multi-task and manage competing priorities.
* Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).
* Ability to solve problems creatively and ethically, while maintaining client-centered, respectful interactions.
* Mental and emotional resiliency is essential.
* Ability to set and maintain professional boundaries.
* Experience using de-escalation and conflict resolution techniques.
*Experience & Knowledge:*
* Minimum of two years' experience delivering community-based services.
* Knowledge of landlord and tenant legislation.
* Familiarity with evidence-based practices, motivational interviewing, and cognitive behavioral techniques.
* Experience building networks with partners and stakeholders, and collaborating across teams or departments.
* Understanding of poverty-related issues, trends, and services, especially as they affect Indigenous populations.
*Additional Requirements:*
* Valid Class 5 driver?s license and access to a reliable vehicle for local travel.
* Criminal Record and Child Intervention Checks (reviewed on a case-by-case basis
