*POSITION: *Addictions Supportive Housing (ASH) Intensive Addictions Case Manager
*LOCATION:* remote and in person (community/ client homes and ADAPT offices)
_*This position will be classified as a 1.0FTE, 37.5 hrs/ week, backfill*_

*SUMMARY*
Delivers clinical and case management services to clients registered with the Addictions Supportive Housing (ASH) Program mainly on an assertive outreach basis by providing clinical assessments and screenings for eligibility, developing and implementing clinically sound treatment plans, and teaching life skills as needed.
*ESSENTIAL DUTIES AND RESPONSIBILITIES*
_ADAPT Addictions Counsellor Duties and Responsibilities: _
* assesses client needs through the use of diagnostic tools and structured interviews;
* in consultation with the Client, develops an individual treatment plan for the client;
* provides individual and/or group counselling to address the clients? needs;
* refers clients to other appropriate support services and agencies according to the clients? needs;
* conducts appropriate case management services with respect to the client?s treatment plan;
* provides consultative services to community agencies, professionals or individuals as required;
* maintains accurate and complete clinical records and service tracking data according to guidelines established by ADAPT, the MOHLTC and/or the funding agency;
* assists in the promotion of ADAPT and its programs to other agencies and the general public;
* provides other services according to the conditions of program funding;
_Addictions Supportive Housing (ASH) Case Manager Duties and Responsibilities:_
* provides support specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, community integration, etc., as defined in the program outline;
* provides counselling/ support and case management services to clients participating in ASH programming;
* participates in clinical screening and admissions screening for the ASH Program;
* conducts case conferences/ consultations with treatment providers, including physicians, other community organizations and other significant persons, as deemed appropriate;
* may assist client and housing partners in locating appropriate rental units for ASH Housing;
* liaise/ advocate with landlords, property managers and housing subsidy providers;
* advocates on behalf of clients;
* travels throughout Halton and Peel Regions;
*EDUCATION*
* College diploma/ university degree in health or social sciences;
* Addictions and Mental Health/ Concurrent Disorders post-graduate diploma is an asset;
*LICENSES, CERTIFICATIONS AND CREDENTIALS*
* Registered Social Service Worker or certification with the Canadian Addiction Counsellors Certification Federation is an asset;
* Current CPR and First Aid certification is required.
* ASIST training or equivalent is required.
* CPI training is required.
* GAINS certification is an asset.
*RELATED WORK EXPERIENCE*
* 2-3 years of experience working with clients 16 years of age and older in the area of addictions, and mental health;
* Individual, family, and group therapy
* Community development and networking
* Record keeping and file management
* Experience in transitional housing case management an asset;
* Experience in: Individual, family, and group therapy, community development and networking, record keeping and file management
