*JOB SUMMARY *
Reporting to the Operations Manager, the Dietitian, with a Diabetes Educator Certificate (CDE) is responsible for the provision of diabetic, dietetic and nutritional services to clients of the South Georgian Bay Community Health Centre (SGBCHC) and the community it serves. The Dietitian takes a lead role in community diabetes and nutrition and working through sustainable change models. The Dietitian works within a multi-disciplinary team to their optimal scope of practice providing group educational sessions and one-on-one appointments.
*MAIN ACTIVITIES *
*Duties and Responsibilities *
* Develops and delivers workshops and presentations across the community to promote healthy eating, food skills development and diabetes management.
* Develop client centered health maintenance plans including those related to diabetes management.
* Plans and prepares healthy eating and food skills resources (e.g., teaching kits, educational resources), (e.g., CHC staff, staff and students of elementary/ secondary schools, health professionals, local food vendors, municipalities, community partners, priority populations and general public).
* Provides consultation regarding the planning and implementation of awareness raising, environmental supports, and policy initiatives to promote healthy eating and/or food skills by providing resources and/or advice.
* Provides advice and information to community members and develops or researches resources to connect community members and other stakeholders with healthy eating and food skills development programs and services.
* Completes dietetic and diabetic assessments to identify client nutritional needs and determine client-centered treatment goals.
* Provides treatment/nutritional care program and develops nutritional resources to address client dietary needs and work towards achieving established therapeutic goals.
* Monitors and re-assesses client progress on an on-going basis, and adjusts treatment program accordingly, to ensure that assessment needs and nutritional goals are addressed.
* Teaches and counsels clients, their family members/care givers/ substitute decision makers (SDMs) regarding nutritional conditions/diseases, client issues/concerns and dietary strategies to improve function, compensate for difficulties and promote nutritional health.
* Advocates for clients as warranted, and facilitates connections between clients and community supports, to assist clients in accessing required resources and in following up with any funding issues.
* Maintains professional competence and knowledge of current practices by participating in professional development and ongoing research.
* Maintains clinical records and documents with client assessment findings, goals, treatment plans and interventions in accordance with SGBCHC?s policies and procedures and as per guidelines established by CDO.
* Protects client confidentiality in accordance with legislation, SGBCHC?s policies and standards established by CDO.
* Teaches and counsels clients, their family members/care givers/ substitute decision makers around diabetes management.
* Other duties as assigned.
*JOB REQUIREMENTS *
*Education*
* Registered Dietitian in good standing with the College of Dietitians of Ontario (CDO).
* Diabetes Educator Certification (or willing to work toward certification)
* 3-5 years of current experience with a focus on primary care in a community setting.
*Knowledge/Experience*
* Knowledge of human nutrition, diabetes management and key health promotion strategies.
* Comprehensive knowledge of the standards of practice and professional guidelines set for the by CDO.
* Practical knowledge and understanding of pertinent legislation.
* Relevant experience providing diabetes, nutritional and dietetic services in a community/health centre environment.
* Knowledge of other community
