Our services will be extended to various locations offering access clinics with expanded hours to better serve our community. </span></p><p><span>Our enhanced care team will offer a range of social and health services focused on illness prevention, health promotion, health education, comprehensive primary care, social prescribing, and population health. initiatives. This will be done through a team comprised of MDs, Holistic Intake and Navigation Counsellors, Peer Support Workers, and RNs. These services will be accessible through various points in the community, making it convenient for residents to receive care. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.</p><p></p><p>Heath Access Thorncliffe Park (HATP) is committed to providing coordinated Health & Wellness services to the Thorncliffe Park community. It is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. HATP is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the Ontario Health- Toronto region.</p><p><strong> </strong></p>Position Summary:<p>The Community Health Registered Nurse, as part of the Primary Care team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health Registered Nurse who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care for the residents or Thorncliffe Park, Flemingdon Park and North York.</p>Responsibilities include:<p>Direct Client Care:</p><ul><li>Participate in providing both access and attachment to primary care within the full scope of a Registered Nurse in an expanded role. Coordinate client care within the health centre and with specialists and other health care agencies.</li><li>Provide telephone advice and triage for requests, abnormal results, screening purposes and health prevention issues.</li><li>Initiate health teaching and health promotion with clients as part of a clinical assessment and group
